Office hours are 8 a.m. to 5 p.m. Monday through Friday.
New Patients
If you are new to this office location, we have a number of forms that, for regulatory reasons, we must have you complete. Therefore, please arrive 30 minutes prior to your appointment time.
Arrival
Upon arrival, please sign the patient register. At each visit, please notify us of any changes in name, address, telephone number or insurance. We will make every effort to be on time for your appointment.
Delays
Please know that there are situations when extremely ill patients or emergencies delay physicians. Please keep in mind that if you are late, other patients scheduled that day may be kept waiting. Call us if you expect to arrive late or are unable to keep an appointment.
Appointment
Appointment length will vary. New patients can expect appointments to last at least one hour. During that time, we will review your history, provide a complete examination and discuss your case.
Children
Health regulations require that you leave children at home or that children remain with an adult in the waiting room.
